New 2005 USSF Registration Form Available (Entry Clinics Only)
There is a new, on-line PDF version of the USSF entry referee registration form for 2005 which has been modified specifically for the DCV instructional program. Attendees signed up for any entry level class (primarily grade 8 or 9) should use this form whenever possible to ensure that the registrations are neat and easily readable by machine scanning in Chicago. An instruction sheet is included at the front of the form to assist students in filling it out.

If you are involved in an incident as a referee (i.e., Referee, AR, or 4th Official) that requires you to file a report with the State Association - for referee assault, referee abuse, or coaches misconduct - you need to file a report.
